イントロダクション～人口減少と経済成長の停滞を背景にしたヘルスケアシステム 近未来の医師について予想するために，まずは将来のヘルスケアシステムの大まかなデザインについて想像してみたい。 西村[1]によると日本の人口構成の今後の基調は少子高齢化と人口減少だが，特に実際の高齢者人口の予測をみてみると，90歳以上の人口の激増が目立つ。また，年齢構成別の医療費をみると，90歳以上の一人あたりの医療費は他のどの世代よりも高い。従って，現時点でのヘルスケアシステムや医療内容が継続する限り，おそらく医療費は上昇を続けるだろう。また，日本の経済自体が低成長の時代に入ってきており，加えて人口全体の減少が進行することはほぼまちがいないことである。これまでの日本の医療環境の最大の変化は高齢化及び低成長であり，医療自体の目的や価値が大きくパラダイム転換する可能性が大きい。 そして，65歳定年制で，それ以降を保証さ
